Position Highlights:

• Provide quality care with the time needed to deliver your best care
• Support team includes a dedicated Nurse, a Referral Coordinator, and a Health Coach for patients.
• Panel size will not exceed 575 patients

Compensation:

• Base Salary with incremental increases
• 30+ days off per year 
• Possible 4.5 day work week
