Outpatient position for a federally qualified community health center has a need for a Family Medicine provider in southern Connecticut.

Practice Details

• 50% of the time you would be working on the medical mobile unit
• Work with all ages in Adult Medicine Department and Pediatrics department
• Recognized level 3 Patient-Centered Medical Home
• Providing services in medical, dental, and behavioral health to 14,000 patients
• On-site pharmacy and lab
